WINCHESTER, Va. - Shenandoah University Head Athletics Trainer Mike Kotelnicki, ATC announced Wednesday that athletic training forms for all returning and new student-athletes are available for submission.
Included in this packet are forms for medical participation, general physical and for Sickle Cell Trait testing.
Per NCAA regulations, all student-athletes must be tested for the Sickle Cell Trait or submit proof that they had the testing done at birth.
"Again this year, the medical participation forms must be completed online," Kotelnicki said.
"To participate in intercollegiate athletics, all of the forms must be completed. We know that student-athletes want to practice as soon as they are able, and so we wanted to make these forms available for submission as soon as possible. Only accepting forms submitted online also makes this process more efficient." Kotelnicki said.

All new student-athletes at SU must fill out the Sickle Cell Trait form. Returning student-athletes have this form on file.
Returning student-athletes who have had a change in prescription medication that necessiates an exception are asked to fill out those two forms.
The Sickle Cell Trait, change of insurance and medical exception forms must all filled out online.
